Virginia Commonwealth University Health Systems is seeking a Patient Flow Coordinator to optimize bed management and transfer coordination across affiliated hospitals.

The role requires RN licensure in Virginia, BLS certification, and three years of acute care experience; a BSN is encouraged. The position involves 36 hours per week with rotating shifts including weekends/holidays and supports demand-driven bed placement.

Qualifications

• Diploma or Associate Degree from a professional nursing program recognized by the state's Board of Nursing.
• All Patient Flow Coordinators without a Bachelor's degree in Nursing must enroll in RN to BSN within 2 years and complete within 5 years.

Responsibilities

• Coordinates patient flow throughout each hospital based on clinical status to provide optimal care.
• Serves as a clinical and administrative resource for bed assignments and complex patient care situations.
• Collaborates with physicians, nurse managers, charge nurses, and bed management to address flow barriers.
• Triages and coordinates transfer referrals into VCUMC and with Lifecom for outbound transfers.
• Manages psych intake referrals in partnership with psychiatric admission representatives.
• Handles all in- and outbound calls supported by One Call Center.
• Ensures continuity of care by coordinating transfers and transports.
• Assists admitting physicians in determining appropriate level of care based on admission criteria.
