Conifer Health Solutions, LLC is seeking an ER Screener and field Visit representative for the Palm Beach region. The role involves screening self-pay patients at hospital bedside for eligibility in governmental and non-governmental programs, identifying payor sources, and guiding patients through applying for benefits.

Position requires conducting interviews, documenting activities, performing financial clearance, and coordinating with hospital staff.

Qualifications

• Screen patients for government and non-government programs.
• Obtain and assist with Confidential Financial Statement forms.
• Help patients apply for benefits they may be eligible for.
• Record complete documentation of activities in patient accounting.

Responsabilités

• Conduct interviews with patients/family members.
• Maintain documentation of activities on accounts.
• Perform financial clearance including collections.
• Follow up on Government applications and coordinate with hospital staff.
• Conduct field visits to patient homes for skip tracing or document assistance.
• Notify case management, social services, and admissions of findings.
