Soleo Health is seeking a Full-time Infusion Care Coordinator (Patient Ambassador) to support our Specialty Infusion Pharmacy in Eden Prairie, MN. The role emphasizes data entry for billing, coordinating nursing schedules, and delivering a seamless patient experience.

Responsibilities include handling referrals, verifying benefits, and maintaining strong communication with patients and providers, all in a 40-hour, on-site workweek.

Qualifications

• Prior healthcare scheduling experience required.
• Home infusion experience preferred.
• On-site work with patients in a clinical setting.
• Strong communication skills.
• Ability to prioritize and multitask.
• Basic computer skills: Excel, Word, Outlook.
• High school diploma or equivalent.

Responsibilities

• Enter clinical information into the management system for billing.
• Coordinate scheduling activities for the nursing department.
• Receive referrals and collect insurance information.
• Contact referral sources and patients to obtain required information.
• Provide exceptional customer service to internal and external customers.
• Coordinate and plan scheduling ensuring accuracy and efficiency.
• Schedule patient visits to support timely medication dispensing.
