Soleo Health is seeking a Full-time Infusion Care Coordinator (Patient Ambassador) to support our Specialty Infusion Pharmacy in Woodridge, IL. The role focuses on data entry, scheduling coordination for the nursing team, and ensuring a positive patient experience.

On-site position with no remote work; prior healthcare scheduling experience and strong communication are required. Healthcare infusion familiarity is preferred, along with excellent organization and customer-service skills.

Formación

• Prior healthcare scheduling experience required
• Home infusion experience preferred
• Ability to work on-site and interact directly with patients in a clinical setting
• Strong communication skills
• Ability to prioritize and multitask
• Basic computer skills including Microsoft Excel, Word, Outlook
• High school diploma or equivalent required

Responsabilidades

• Receive medication referrals and collect insurance information through various methods.
• Proficient data entry and generating daily office communications.
• Contact referral sources, patients, or prescribers' offices to obtain additional information required for verification of benefits or prior approvals.
• Place outbound calls to patients or doctor’s offices to notify them of care status.
• Collaborate with prescribers to facilitate payor denial appeals.
• Act as a single point of contact for all referral activity with internal customers, manufacturers/HUBs, patients, prescribers, and referral sources.
• Provide exceptional customer service to external and internal customers, resolving requests in a timely and accurate manner.
• Coordinate and plan scheduling activities for the nursing department, ensuring accuracy and efficiency.
• Meet with patients in person following AIS visits to schedule their next appointment prior to departure.
