Soleo Health is hiring a Full-time Patient Infusion Care Coordinator (Patient Ambassador) to support our Specialty Infusion Pharmacy in Dublin, OH. The role focuses on data entry for billing, coordinating nursing scheduling, and ensuring a seamless patient experience across inter-agency care coordination.

The candidate will manage referrals, verify benefits, and serve as the primary contact for referral activity while delivering exceptional customer service in a fast-paced clinical setting.

Qualifications

• High school diploma or equivalent required.
• Prior healthcare scheduling experience required.
• Home infusion experience required.
• Strong communication and multitasking abilities.
• On-site collaboration with nursing and referral teams.

Responsibilities

• Receive medication referrals and collect insurance information.
• Proficient data entry and generate daily office communications.
• Contact referral sources, patients, or prescribers to obtain info for verification or approvals.
• Place outbound calls to patients/physicians to notify care status.
• Collaborate with prescribers to facilitate payor denial appeals.
• Act as single point of contact for referral activity with internal/external partners.
• Provide exceptional customer service and resolve requests promptly.
• Notify patients about financial responsibility and coverage for services.
• Coordinate scheduling for the nursing department and ensure efficiency.
• Meet with patients after AIS visits to schedule next steps.
