Soleo Health, an innovative national provider of complex specialty pharmacy and infusion services, is seeking a Full-time Infusion Care Coordinator (Patient Ambassador) in Eden Prairie, MN. This role focuses on data entry for billing, scheduling for nursing teams, and coordinating inter-agency care to ensure a seamless patient experience.

Qualifications

• Prior healthcare scheduling experience required.
• On-site work and patient interaction in a clinical setting.
• Strong communication skills.
• Able to prioritize and multitask.
• Basic computer skills including Excel, Word, Outlook.
• High school diploma or equivalent required.

Responsibilities

• Receive medication referrals and collect insurance information.
• Proficient data entry and generating daily office communications.
• Contact referral sources, patients, or prescribers to obtain information for verification of benefits or prior approvals.
• Place outbound calls to patients or doctors to notify care status.
• Collaborate with prescribers to facilitate payor denial appeals.
• Act as a single point of contact for all referral activity with internal customers, manufacturers/HUBs, patients, prescribers, and referral sources.
• Provide exceptional customer service to external and internal customers, resolving requests in a timely and accurate manner.
• Ensure notification of patients regarding financial responsibility, benefit coverage, and payor authorization for services.
• Serve as main point of contact for nursing department, multi-tasking and staying calm under pressure.
• Coordinate and plan scheduling activities for the nursing department.
• Demonstrate knowledge of the geographical area for efficient scheduling and coordination of nursing visits.
• Communicate patient needs for external agency support to ensure timely care.
• Collaborate with internal departments and external partners to promote and maintain relationships.
• Support client satisfaction to ensure account retention.
• Meet with patients after AIS visits to schedule next appointment.
• Confirm patient understanding of next visit and accurate scheduling for timely dispensing.
• Solicit feedback via brief satisfaction surveys.
• Serve as a key touchpoint for enhancing the in-clinic patient experience.
