Job summary

Soleo Health is seeking a Full-time Infusion Care Coordinator (Patient Ambassador) to support our Specialty Infusion Pharmacy in Woodridge, IL. Join us in Simplifying Complex Care!

The Patient Ambassador will manage data entry of clinical information, coordinate nursing schedules, respond to referrals, and ensure a seamless patient experience from AIS visits through follow-up appointments.

Qualifications

• Prior healthcare scheduling experience required.
• Home infusion experience preferred.
• Ability to work on-site and interact directly with patients in a clinical setting.
• Strong communication skills.
• Ability to prioritize and multitask.
• Basic computer skills including Microsoft Excel, Word, Outlook.
• High school diploma or equivalent required.

Responsibilities

• Receive medication referrals and collect insurance information.
• Proficient data entry and generating daily office communications.
• Contact referral sources, patients, or prescribers' offices to obtain information for verification of benefits or prior approvals.
• Place outbound calls to patients or doctor’s offices to notify them of care status.
• Collaborate with prescribers to facilitate payor denial appeals.
• Act as a single point of contact for all referral activity with internal customers, manufacturers/HUBs, patients, prescribers, and referral sources.
• Provide exceptional customer service to external and internal customers, resolving requests promptly.
• Ensure notification of patients regarding financial responsibility, benefit coverage, and payor authorization for services.
• Serve as the main point of contact for the nursing department and coordinate scheduling for patient care teams.
• Coordinate and plan scheduling activities for the nursing department, ensuring accuracy and efficiency.
• Demonstrate knowledge of the geographical area for efficient scheduling and coordination of nursing visits.
• Communicate patient needs for external agency support to ensure timely care.
• Collaborate with internal departments and external partners to maintain relationships across lines.
• Support client satisfaction to ensure account retention.
• Meet with patients in person after AIS visits to schedule next appointments.
• Confirm patient understanding of their next visit and ensure accurate scheduling for timely medication dispensing.
• Inquire about patient appointment experience and collect a brief satisfaction survey.
