Job summary

Ascension Living in Indianapolis, IN is seeking an RN for full-time Day shifts with weekend coverage as needed. The role provides direct participant care including triage, blood draws, and wound care as part of an interdisciplinary team.

You will participate in care planning, IDT meetings, and education for participants and families; travel to residences or facilities may be required. Licensing and BLS are mandatory to hire.

Qualifications

• BLS Provider certification within 1 month (30 days) of hire or transfer date.
• Registered Nurse licensure required prior to hire.
• Diploma from an accredited school/college of nursing or equivalent professional licensure.

Responsibilities

• Provide direct participant care including acute triage, blood draws and wound care.
• Assist care team with provision of participant care including after-hours/weekend on-call rotation.
• Attend IDT care plan meetings and participant/family meetings as applicable.
• Develop and provide education to participants, caregivers, and associates including end-of-life discussions; may require travel to participant residences or facilities.

How you’ll make an impact in this role

• Provide direct participant care including acute triage, blood draws and wound care. Assists the care team for provision of participant care including after hours/weekend on-call rotation.
• Participate in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care and update changes as necessary.
• Attend Interdisciplinary Team (IDT) care plan meetings and Participant/Family meetings as applicable. May perform assessments in a variety of settings including PACE Clinic and/or Adult Day Center.
• Develop and provide education to participants, caregivers, and associates as necessary including end of life discussions. May require travel to participant's place of residence, long term care and assisted living facilities.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Registered Nurse obtained prior to hire date or job transfer date.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
