Ascension Health in Indianapolis, IN is seeking a full-time Home Care Scheduling Specialist to support the Home Care team and St. Vincent PACE during daytime hours.
You will coordinate external medical appointments, log all details in the electronic medical record, and maintain the external provider contact directory in line with site workflows and compliance.

Schedule: Days | Full-time
Scheduling & Referrals: Coordinate external medical appointments, referrals, and schedule changes for assigned patients.
EHR Documentation: Log all appointments, special instructions, and patient details accurately into the electronic medical system.
Provider Contact Management: Maintain and regularly update the external provider contact directory.
Protocol & Compliance: Follow site-specific workflows and keep documentation up to date to ensure compliance.
High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
