Job summary

Bridgeport Hospital, part of Yale New Haven Health, seeks a Patient Care Associate to support nursing staff under RN supervision. You will collect vitals, document patient data, and participate in care planning while coordinating with the care team to meet patient needs. CNA or EMT credentials and BLS are required, with two years of direct patient care or equivalent training. The role requires flexibility to work days/evenings and travel between Fairfield and Milford.

Qualifications

• High school diploma or GED required.
• CNA credential, EMT, or other clinical experience in a healthcare setting.
• Two years direct patient care experience or equivalent education.
• Must complete Patient Care Associate Competency Based Training and Practicum.

Responsibilities

• Under the RN, perform tasks to obtain and report information to assist the nurse in assessment.
• Obtain and record vital signs as ordered.
• Assist in safe and efficient delivery of patient care.
• Identify patients following policy and procedures.
• Collaborate with multidisciplinary team to address patient/family needs.
• Communicate with patients, families, and staff; cover BA desk when needed.
• Relocate and rotate shifts to meet patient care needs.
• Deliver medications from pneumatic tube system to appropriate staff.
