Job summary

Yale New Haven Health's Bridgeport Hospital seeks a Patient Care Associate to support RN-led patient care. You will perform assigned tasks, record vital signs, and help ensure accurate patient identification.

Collaboration with the care team and effective communication with patients and families are essential. Requirements include a high school diploma or GED and CNA/EMT or equivalent clinical experience; BLS certification is required or obtained within 1 month of hire.

Qualifications

• High school diploma or GED is required or equivalent.
• Certification or clinical healthcare experience such as CNA/EMT is preferred.
• Successful completion of orientation and English proficiency.

Responsibilities

• Under RN supervision, perform selected tasks to assist in patient assessment, planning and evaluation.
• Obtain and record vital signs as ordered or delegated by the RN.
• Assist in safe and efficient delivery of patient care and ensure proper patient identification.
• Collaborate with the multidisciplinary team to address patient/family needs with cultural sensitivity.
• Communicate effectively with patients, families, and staff; cover BA desk when needed.
• Relocate and rotate to meet patient care needs and assist with pneumatic tube medication delivery.
