Qualifications

• High School diploma or equivalent is required.
• 1 year experience in a health care setting preferred.
• Ability to communicate effectively in English both verbally and in writing.

Responsibilities

• Responds to patient call within 10 minutes.
• Assists with essential skills for patient care: vitals, daily weight, I&O, hygiene, meal assistance, mobility.
• Maintains safe environment and safe operation of patient-care equipment; ensures accessibility of call lights and emergency equipment.
• Documents all care provided, including vital signs, intake/output, meal percentages, equipment used, and patient care.
• Communicates plan of care to patient and family; collaborates with RN and other staff.
• Ensures confidentiality of patient records and maintained privacy of information.
• Performs other related duties as assigned.
