Qualifications

• LPN licensed by the state of employment preferred.
• At least one year of home care/hospice experience preferred.
• Requires excellent interpersonal and communication skills with all levels of management.
• Strong organizational skills and ability to prioritize tasks and manage time efficiently.
• Computer experience necessary to perform responsibilities.
• Good working knowledge of the patient referral process.

Responsabilités

• Receives/responds to incoming calls from referral sources and patients; obtain patient orders for processing referrals.
• Establish patient and employee schedules by coordinating patient needs and staff availability; process scheduling changes.
• Input all essential patient and staff schedule information in EMR accurately and timely.
• Coordinate schedules and relay them to the appropriate party to ensure optimal patient care.
• Maintain current records of staff availability and assign personnel to patient needs.
• Communicate problems or concerns about patient services or staff performance to the appropriate person.
• Reschedule staff schedules due to illnesses or emergencies and inform clinical staff of changes.
• Produce reports as requested by appropriate personnel.
• May attend PI Utilization Review Committee quarterly or as scheduled.
• Organize, prioritize and execute work assignments efficiently.
• Attend case conferences, inservices, seminars and other meetings as assigned.
• Maintain confidentiality of business and personnel information.
• Maintain a professional attitude and foster teamwork among staff.
• Avoid personal matters from interfering with work time.
• Maintain professional appearance at all times.
• Provide proper notification of absence or tardiness without abuse.
• Participate in special projects or overtime as requested.
• Follow all agency policies and procedures.
