Job summary

Heart to Heart Hospice seeks a detail-focused Hospice Aide Scheduler/Coordinator to organize patient visits and coordinate with care teams. You will ensure timely documentation and maintain engagement withAdmissions/Intake to align staffing with patient needs.

The role relies on strong communication, organizational skills, and reliable transportation. Experience with EMR and CNA certification are preferred, with at least one year in hospice or related care settings.

Qualifications

  • High School Diploma preferred.
  • Experience with EMR is a plus.
  • CNA licensure preferred.
  • One year experience as a hospice aide or nursing assistant preferred.

Responsibilities

  • Utilize EMR software to schedule clinical staff patient visits.
  • Schedule patient visits according to care plans and staff availability.
  • Confer with Patient Care Manager on problems and complaints timely.
  • Update and maintain the schedules on a regular basis throughout the day, enter scheduling data, create schedules.
  • Provide staffing scheduling for sick leave, vacation, long term leave.
  • Verify documentation of information such as completion of visits, frequency & duration of visits.
  • Schedule new patients for identified services and maintains a close connection with Admissions/Intake for communication of admission needs and staff availability.
  • Ensure scheduled visits are entered timely.
  • Order and maintain patient supply inventory; possibly help aides load in their cars.
  • When frequency is incorrect or missed, notify PCM.
  • Oversight and tracking of staff in regard to punctuality, assignments and reliability.
  • Communicate and report to Patient Care Manager regarding tardiness, unplanned, unscheduled Aide Call Ins and other confidential findings.
  • Timely submission of all documentation.
  • Conduct all business activities in a professional and ethical manner.
  • Performance of other duties as required.
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.

Hospice Aide Scheduler/Coordinator
QUALIFICATIONS AND EDUCATION REQUIREMENTS
  • High School Diploma Preferred.
  • If an aide, has successfully completed one of the following:
    1. Training program/competency eval
      as specified in §418.76(b)(c)of the Medicare regulations
    2. Competency eval program that meets §418.76(c)
    3. Nurse aide training/competency eval program approved by the state and listed in good standing on the state nurse aide registry
    4. State Licensure program that meets requirements of §418.76(b)(c)
  • CHPNA certification a plus
  • Experience in EMR a plus
  • Licensure: CNA preferred.
  • At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health

agency, hospice, or long term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide

Program. One year experience as an aide scheduler preferred.

  • Demonstrates good communication, interpersonal, negotiation and public relations skills
  • Demonstrates organization, assertiveness, flexibility, cooperation in performing job responsibilities
  • Must have current driver's license
  • Must have reliable transportation and provide ongoing valid and current auto liability insurance
