Job summary

Penn Medicine seeks a COE Care & Outreach Coordinator to be the single point of contact for treating physicians, clinicians, and patients, providing resources and access to clinical and supportive care services within UPHS and the community.

The role includes coordinating monthly patient visits, coordinating information with interdisciplinary providers, expediting scheduling for follow-up and acute care, and representing the COE at community events to expand reach.

Qualifications

• H.S. Diploma/GED required.
• Associate of Arts or Science or related field preferred.
• Equivalent experience in behavioral health considered.

Responsibilities

• Coordinate monthly patient meetings to assess needs and compliance.
• Link patients with social services, health and medications via interdisciplinary team.
• Facilitate appointments and expedite follow-ups within UPHS.
• Coordinate community outreach to promote COE services and build partnerships.
• Prepare concise patient progress summaries and document recommendations.
• Monitor care delivery and contribute to data collection for state requirements.
