Job summary

Penn State Health is seeking a dedicated care coordinator to manage the individualized care continuum for a specific patient population. You will advocate for patients, educate families, and coordinate with the medical team to ensure seamless care across settings and reduce barriers to access.

The role emphasizes guiding patients through the complex healthcare system, assessing biopsychosocial needs, and improving outcomes while supporting a patient-centered experience.

Qualifications

• Bachelor's degree in a human services field.
• Minimum two (2) years related experience.
• Non-Violent Crisis Prevention training required prior to end of orientation.
• Basic Life Support (BLS) required prior to completion of orientation.
• PA Act 31 Child Abuse Training Certificate.

Responsibilities

• Coordinate the individualized care continuum for patients and achieve quality, cost-effective and patient-focused outcomes.
• Advocate, educate, consult, lead and serve as a community ambassador for a specific patient population.
• Provide consistent care coordination throughout the continuum of care and assess physical, psychological and social needs of the patient.
• Guide patients and families through the complex healthcare system and eliminate barriers to care/outcomes.
• Coordinate efforts of the medical team to facilitate continuity of care and enhance patient satisfaction.
