Job summary

Penn Medicine is seeking a Patient Coordinator for Care Management to support the Penn Partners Care Management Program. You will coordinate services and connect with patients to advance positive health outcomes through telephonic, electronic, and in-person contacts.

The role emphasizes collaboration with the Care Manager, PCPs, social workers, pharmacists and other care providers, with documentation in PennChart and coordination of transportation and follow-up within the care plan.

Qualifications

• H.S. Diploma/GED and 3+ years of relevant health care experience in a similar role required
• Associates Degree and 1+ years health-related or science area of study preferred
• Bachelor's Degree and 0-1 years health-related or science area of study preferred

Responsibilities

• Documents in PennChart using the Care Management Navigator regarding coordination activities
• Coordinates patient needs (e.g. social service, health and medication) with interdisciplinary providers
• Interacts with patient service representatives or schedules follow-up for ED, hospital follow-up and acute care appointments
• Monitors delivery of care through documentation of interventions and follow-up activities
• Collaborates with Care Manager, Community Health Worker, Social Worker or Pharmacist to support patient goals and access to resources
