Tryon Medical Partners is seeking a Patient Care Coordinator for Uptown to ensure the physician’s plan of care is accurately completed and delivered on time. You will coordinate referrals with physicians, hospitals, and community services, schedule follow-up visits, and assist patients with questions about delivery system access.

This full-time role supports the Primary Care team at Uptown, with standard hours Monday–Friday 8 am–5 pm.

Formación

• Minimum of one-year medical office or healthcare facility experience.
• E.H.R. and Scheduling experience.
• Experience serving customers in person and on the phone.

Responsabilidades

• Coordinate and schedule referrals and ancillary appointments.
• Follow-up on referrals and appointments to ensure timely completion.
• Schedule physician return appointments and manage forms/requisitions as needed.
• Obtain and send patient records to support seamless care delivery.
