Job summary

Neighborhood Healthcare is seeking a Patient Care Coordinator (Temporary) in Riverside, CA. The role supports Primary Care Practice teams to improve patient health outcomes and patient access to services. This position is a temporary full‑time assignment with an anticipated end of April 2027.

You will manage patient panels, coordinate care, and assist with scheduling, recalls, and communications via the patient portal and phone. A MA certificate and BLS are required upon hire.

Qualifications

• High school diploma or GED required.
• Medical Assistant Certificate/Diploma required.
• Current Basic Life Support (BLS) certification required upon hire per AHA guidelines.
• One year of medical assistant experience preferred.
• Non-profit community health clinic experience preferred.

Responsibilities

• Coordinate and support PCP teams to improve patient health outcomes.
• Provide panel management for preventive and health maintenance follow-ups.
• Contact new patients to welcome them and explain the medical home.
• Schedule patients with chronic conditions for follow-ups.
• Submit prior authorizations for medications and DME as needed.
• Assist medical staff and patients as needed.
• Support weekly team huddles and monthly MA meetings.
