Job summary

Lifekind Health in Riverside, CA is seeking a Care Coordinator to join our team. You will ensure patients receive comprehensive and continuous healthcare services tailored to their individual needs, coordinating care across multidisciplinary teams.

The ideal candidate has at least 2 years of experience in care coordination, strong knowledge of healthcare systems, and excellent communication. Bilingual abilities are a plus as you support diverse patient populations.

Qualifications

Two+ years in care coordination or related healthcare role.
Experience with healthcare systems, patient care processes, and community resources.
Bilingual abilities preferred; strong communication with diverse patient populations.

Responsibilities

Assess patient needs and develop individualized care plans with healthcare professionals.
Coordinate appointments, treatments, and follow-up care for continuity.
Serve as the primary contact for patients and families, providing education and support.
Facilitate communication among patients, providers, and community resources.
Monitor progress and update care plans; document interactions and outcomes.
Identify barriers to care (transportation, finances, language) and address them.
Maintain compliance with privacy laws and organizational policies.
Participate in interdisciplinary team meetings to improve service delivery.
