Job summary

Alliance Family Health Center, Inc. seeks an LPN Care Coordinator to support disease management, referral coordination, and hospital follow-up for high‑risk patients within the PCMH framework.

You will promote team-based care, partnering with patients, families, community resources, and physicians to improve health outcomes. Qualifications

• Two years of clinical experience minimum.
• LPN/RN license required.
• CPR certification required.

Responsibilities

• Use data systems to monitor gaps in care and oversee scheduling of patient appointments for needed services.
• Coordinate continuity of care with external healthcare organizations, including hospital follow-up and referrals.
• Proactively reach out to high-risk patients, obtain consent, and manage patient care.
• Conduct monthly touch points with patients to evaluate goal attainment and health management.
