PALM Health, LLC in St. Louis, MO, seeks a Patient Liaison to deliver a highly personalized, coordinated patient experience. Working closely with an integrative medicine physician, this role ensures seamless communication, follow-through, and continuity of care.

The ideal candidate will have a healthcare background, strong organizational skills, and the ability to interpret physician recommendations while supporting patients through individualized care plans.

Qualificações

• Bachelor's degree in social work, healthcare administration, health sciences, psychology, nursing, or a related field; equivalent relevant professional experience may be considered.
• Three or more years of relevant professional experience in healthcare, patient services, or clinical coordination.
• Experience working directly with physicians in a hospital, outpatient clinic, specialty practice, or concierge medicine environment.
• Previous professional experience in a healthcare setting, preferably in patient care coordination, medical social work, patient navigation, clinical practice support, or a comparable role.
• Strong understanding of healthcare terminology, medical documentation, and the coordination of patient care.
• Ability to interpret and accurately communicate documented physician recommendations, recognizing the boundaries between patient education and independent clinical advice.
• Exceptional organizational skills, attention to detail, and the ability to manage competing priorities in a dynamic clinical environment.

Responsabilidades

• Review physician documentation, communicate recommendations, and ensure alignment with patient care plans.
• Coordinate services and facilitate communication between patients, providers, and the PALM Health team.
• Keep the physician informed and prepared for each appointment to support smooth visits.
• Assist patients in understanding and navigating individualized care plans within an integrative practice.
