Job summary

Axil Health in Raleigh, NC is seeking patient-focused Care Navigators to join a small, mission-driven team. You will contact assigned patients on a structured schedule, review remote monitoring data, document interactions, and escalate concerns to a pharmacist according to established protocols.

Qualified CMAs/CCMAs/RMA/NCMA or LPNs with an active NC license are required. We offer health, dental, vision insurance, disability, a 401(k), 15 days PTO, paid holidays, and paid training.

Qualifications

• Certified Medical Assistants (CMA, CCMA, RMA, NCMA, or equivalent national certification) or Licensed Practical Nurses (LPN) with an active NC license are required.
• Ability to document patient interactions and coordinate care with pharmacists and clinicians.

Responsibilities

• Reach out to assigned patients on a structured schedule.
• Review remote monitoring dashboards and proactively contact patients when data trends.
• Document patient interactions using streamlined tools.
• Escalate clinical concerns to on-team pharmacist per protocols.
• Assist to enroll newly eligible patients into the program.
