Axil Health is seeking a Patient Care Navigator- Certified Pharmacy Tech to join our care team. You will work closely with clinicians to monitor patient progress and support day-to-day care coordination.

You will reach out to patients, review dashboards, document interactions, escalate concerns to pharmacists, and help enroll newly eligible patients into the program. The role offers a structured schedule and opportunities to grow within our interdisciplinary team.

Responsabilités

• Reach out to your assigned patients on a structured schedule, some weekly, some monthly, based on their condition and risk level
• Review remote monitoring dashboards and proactively contact patients when data is trending in the wrong direction
• Document patient interactions using tools (less typing, more connecting)
• Escalate clinical concerns to an on-team pharmacist using clear protocols
• Assist to enroll newly eligible patients into the program
