Job summary

Axil Health in Raleigh, NC is seeking patient-focused Care Navigators to join a small, mission-driven team. You will contact assigned patients on a structured schedule, review remote monitoring data, document interactions, and escalate concerns to a pharmacist according to established protocols.

Qualified CMAs/CCMAs/RMA/NCMA or LPNs with an active NC license are required. We offer health, dental, vision insurance, disability, a 401(k), 15 days PTO, paid holidays, and paid training.

Qualifications

About Axil Health

We partner our clinical pharmacist with physician practices and build care teams around them; navigators, technology, and the infrastructure to actually manage patients between visits, not just during them.

We currently support over 9,000 patients across 25+ practice locations, including partnerships with Corporate Medical Providers across the Carolinas and Virginia. Our outcomes are real-our monitored patients reach blood pressure goals, achieving measurable A1C improvement, and documented cost savings.

What you'll do day-to-day:

• Reach out to your assigned patients on a structured schedule, some weekly, some monthly, based on their condition and risk level
• Review remote monitoring dashboards and proactively contact patients when data is trending in the wrong direction
• Document patient interactions using tools (less typing, more connecting)
• Escalate clinical concerns to an on-team pharmacist using clear protocols
• Assist to enroll newly eligible patients into the program

Requirements

Qualifications Needed:
