Job summary

Roswell Park Comprehensive Cancer Center is seeking a Case Manager to coordinate patient care, advocate for patients, and ensure timely delivery of services across the continuum. Join a collaborative team in the Case Management department.

Qualifications

• Bachelor of Science in Nursing or equivalent with clinical experience in Utilization Management/Discharge Planning, Home Health Care or Case Management.
• Associate's degree in Nursing with several years of relevant experience.
• Nursing diploma with several years of relevant experience.

Responsibilities

• Coordinate patient care from admission through discharge and follow-up.
• Collaborate with patients, families, physicians, care teams, and payers to ensure appropriate levels of care.
• Advocate for patients and ensure high-quality, efficient, and compassionate care.
