Qualifications

Responsibilities

• Provide outreach in health fairs, clinic outreaches, and targeted lists.
• Deliver culturally appropriate health education and information using education tools within the EHR.
• Assist clients with scheduling appointments and completing applications for eligible programs.
• Administer screening tools to identify individuals needing mental health or substance abuse services.
• Coordinate care by tracking patients and assisting with services outside the practice.
