Job summary

La Clinica de Familia Inc is seeking a CCBHC Patient Specialist to provide a full range of professional nursing services, including assessment, planning, implementation, and coordination of patient care in accordance with individualized Comprehensive Care Plans.

Under supervision of the Clinical Team Lead, you will work with a multidisciplinary team to improve health outcomes, reduce barriers to care, and promote recovery and wellness, addressing social determinants of health.

Qualifications

• Bachelor's degree in health-related field.
• Nursing or Social Work degree preferred.
• Minimum one year of clinical care experience.
• Community Health Worker Certification or ability to obtain within 3–6 months of employment.

Responsibilities

• Provide full range of professional nursing services including assessment, planning, implementation, and evaluation of patient care.
• Coordinate patient care within the CCBHC team and with social services.
• Assist patients navigating behavioral health, physical health, and social service systems.
• Identify barriers to care and connect patients with community-based resources.

Job Summary

The CCBHC Patient Specialist provides a full range of professional nursing services, including assessment, planning, implementation, evaluation, and coordination of patient care in accordance with individualized, team-based Comprehensive Care Plans. Under the direct supervision of the CCBHC Clinical Team Lead, the CCBHC Patient Specialist plays a critical role in supporting individuals as they navigate behavioral health, physical health, and social service systems. The CCBHC Patient Specialist will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services.

Job Requirements
