La Clinica de Familia is seeking a Patient Navigator–Community Health Worker to support clients navigating behavioral and physical health, along with social services. You will engage communities, coordinate care, and advocate for culturally responsive, person-centered services within a multidisciplinary CCBHC team.

Requirements include a high school diploma or equivalent, 3 years of direct patient care, and CHW certification or ability to obtain within 3–6 months.

Qualifications

• High school diploma or equivalent required.
• Community Health Worker Certification or ability to obtain within 3–6 months of hire.
• Medical Assistant Certification preferred.
• Experience with diverse populations, including behavioral health or substance use, preferred.
• 3 years of direct patient care experience.

Responsibilities

• Support clients navigating behavioral and physical health and social service systems.
• Engage communities and promote timely, culturally responsive, person-centered services.
• Assist with outreach, care coordination, and advocacy as part of a multidisciplinary CCBHC team.
• Contribute to improving health outcomes and reducing barriers to care.
