Job summary

La Clinica de Familia (LCDF) seeks a CCBHC Patient Specialist to provide a full range of professional nursing services, including assessment, planning, implementation, evaluation, and coordination of patient care within Comprehensive Care Plans. Under supervision of the CCBHC Clinical Team Lead, you will support individuals as they navigate behavioral health, physical health, and social service systems.

The CCBHC Patient Specialist works as part of a multidisciplinary team to improve outcomes

Qualifications

Bachelor’s degree in health-related field; nursing or social work preferred.
Minimum one year of experience in a clinical care setting.
Community Health Worker certification or ability to obtain within 3–6 months.
Experience with diverse populations, including behavioral health or substance use needs.

Responsibilities

Provide a full range of professional nursing services per Comprehensive Care Plans.
Assess, plan, implement, evaluate and coordinate patient care under supervision.
Support individuals as they navigate behavioral health, physical health and social services.
Collaborate with a multidisciplinary CCBHC team to improve health outcomes.
Address social determinants of health and connect patients with community resources.
