La Clinica de Familia is seeking a Patient Navigator–Community Health Worker to support clients navigating behavioral and physical health, along with social services. You will engage communities, coordinate care, and advocate for culturally responsive, person-centered services within a multidisciplinary CCBHC team.

Requirements include a high school diploma or equivalent, 3 years of direct patient care, and CHW certification or ability to obtain within 3–6 months.

Qualifications

• High school diploma or equivalent required.
• Community Health Worker Certification or ability to obtain within 3–6 months of hire.
• Medical Assistant Certification preferred.
• Experience with diverse populations, including behavioral health or substance use, preferred.
• 3 years of direct patient care experience.

Responsibilities

Job Summary

Under the direct supervision of the Clinical Team Lead, the Patient Navigator–Community Health Worker plays a key role in supporting clients as they navigate behavioral health, physical health, and social service systems. The Patient Navigator will focus on patient and community engagement, outreach, care coordination, and advocacy. Ensuring individuals receive timely, culturally responsive and person-centered services. The Patient Navigator will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services.
