Job summary

Lehigh Valley Health Network in the Lehigh Valley region seeks a Care Navigation Partner to educate, outreach, and assist patients in person. The role triages calls from patients, caregivers, and providers, conducts one-to-one meetings, follows up via email or phone, and promotes timely access to care to reduce ER visits and readmissions.

Collaborate with care teams to develop care plans, support quality initiatives, and close care gaps.

Qualifications

• Bachelor's degree in nursing required.
• RN license in Pennsylvania required.
• BLS certification required.
• Master’s degree in nursing preferred.

Responsibilities

• Focus on chronic illness management per care pathways to reduce ER visits and readmissions.
• Builds strong relationships to help patients achieve goals and seamless care.
• Develops and reassesses patient goals with patient and care team.
• Maintain contact with patient and support circle to promote health outcomes.
• Plan and facilitate patient education; ensure understanding of care and interventions.
• Support practice quality efforts and close care-gap initiatives.
• Complete pre-visit planning and assist with community resources.
