Job summary

Lehigh Valley Health Network is seeking a Care Continuum Partner to coordinate patient scheduling, referrals, and transitions of care at the Hazleton location. You will partner with clinical teams to deliver a seamless patient experience across the care continuum.

The role requires strong communication, confidentiality, and multitasking in a fast-paced health system; EMR and scheduling software experience is preferred.

Qualifications

• High School Diploma or GED required.
• Experience interacting with patients in a clinical setting.
• Experience in a complex health system preferred.

Responsibilities

• Coordinate scheduling and referrals across the continuum of care.
• Assist with pre-visit planning and intake to ensure timely appointments.
• Serve as a liaison among patients, families, and clinical staff.
• Maintain patient records and ensure confidentiality.
