Job summary

Lehigh Valley Health Network is seeking a dedicated Registered Nurse to serve as a primary contact for patients and families, coordinating access to care and supporting clinical pathways. The RN will work closely with referring physicians and the care team to ensure seamless services and timely follow-up.

Responsibilities include coordinating treatment, promoting quality improvement, developing patient education materials, and ensuring compliance with policies.

Qualifications

• RN - Licensed Registered Nurse PA.
• Confidentiality of records and information.
• Knowledge of patient education techniques and principles.
• Knowledge of community resources and how to access them effectively.
• Experience with obesity management theories, practices and techniques.

Responsibilities

• Helps patients and family navigate medical care and treatment options.
• Assesses and coordinates delivery of services to expedite progress.
• Collaborates with medical team for follow-up and monitors progress.
• Participates in development and review of quality metrics.
• Develops and implements patient education materials.
• Identifies indicators to monitor patients with marketing and referral outcomes.
• Compiles data and recommends process improvements.
• Reports quality issues to improve efficiency.
• Engages in quality assessment, utilization review and infection control.
• Acts as clinical liaison for new/existing patients.
• Provides referrals and support for follow-up care for uninsured/underinsured.
