Job summary

Arcare is seeking a Patient Care Coordinator to ensure coordinated, patient-centered services across the care team. You will work with patients and families, doctors, and community resources to manage care plans and referrals.

The role emphasizes communication, organization, and advocacy, with a Monday–Friday schedule and a focus on quality care and compliant processes. You will help navigate insurance, transportation, and resources to support seamless patient journeys and improve outcomes.

Qualifications

• High School Diploma or equivalent required.
• 1 year of related experience required.
• Strong understanding of healthcare systems, insurance, and community resources.

Responsibilities

• Oversee the coordination of patients care while serving as a primary contact for patient and their families.
• Assist with scheduling, transportation, and connecting patients to community resources and support programs.
• Track patient progress, while identifying and resolving care coordination gaps to enhance patient outcomes.
• Participate in ongoing training related to care coordination and quality.
