Job summary

Duties include front desk operations, patient access, referral follow-up, preventive outreach, transportation when needed, and ensuring HRSA program requirements are met.

Qualifications

• Bachelor's degree in Social Work or 1+ year experience as CDCA, LCDC I or II
• Must have a valid driver's license and must pass a background check and drug test.
• Experience supporting clinical workflows and patient data documentation preferred.
