Job summary

Advantage Healthcare Services is seeking a Community Health Liaison to support behavioral health patients in clinic, parole, and home-based settings. You will coordinate care with clinical and pharmacy teams, provide patient support services, and help optimize medication adherence and health outcomes.

The role requires a high school diploma, 1–2 years in a community-based setting, and experience in medical assistance, community health work, or related fields.

Qualifications

• Work in a multi-cultural setting with diverse patients.
• Proficient in Microsoft Office applications.
• Understanding of behavioral health concepts and patient needs.
• Excellent oral and written communication skills.

Responsibilities

• Conduct Enhanced Clinical Services to AHCS patients.
• Offer MTM and other clinical services to optimize care.
• Attend in-person patient visits for enhanced services programs.
• Coordinate care with patients' healthcare providers and care team.
• Improve patient adherence and access to prescriptions.
• Attend meetings with clinical team members and complete reporting.
