Job summary

The candidate will screen eligibility, enroll patients, and help organize volunteer navigators while maintaining accurate documentation and data in Salesforce.

Qualifications

• Bachelor of Science and 1–2 years of experience required.
• Extremely organized and detail oriented with strong communication skills.
• Experience in public health, health care or social work is a plus.

Responsibilities

• Conduct patient intake and enrollment, screen patients and explain CSH services.
• Track patient progress and ensure timely access to surgical care.
• Recruit and organize volunteer Personal Patient Navigators (PPNs) and support dyads with navigation steps.
• Input data into CSH Salesforce and respond to operational queries promptly.
• Regularly report progress to managers and maintain accurate patient documentation.
