Qualifications

Responsibilities

• Process patient intake and enrollment, screen eligibility, and document patient information.
• Recruit volunteer Personal Patient Navigators and support their navigation steps.
• Input data into CSH Salesforce and respond to operational queries.
• Regularly report progress to managers and maintain patient documentation.
• Coordinate with primary care and ensure timely access to services.
