Job summary

Qualifications

• High School diploma or GED required.
• Minimum 1 year of experience in the medical field utilizing Electronic Health Records (EHR) required.
• Additional 1 year of experience in a clinical setting, Family Practice or Internal Medicine preferred.

Responsibilities

• Coordinate the registration of new patients by obtaining necessary information, including past medical records, abstractions and demographics.
• Coordinate patient referrals to specialists and collect/abstract referral records when visit is completed.
• Identify and reconcile gaps of care for patients, including quality measures that have not been met.
• Ensure all necessary documents are properly saved in patients’ electronic health record.
