Job summary

Mount Sinai Morningside is seeking a Services Management Coordinator to coordinate patient care within assigned programs. You will act as a liaison among physicians, patients, nurses, and case management, providing administrative support to providers and the care team.

The role focuses on educating patients about program benefits, coordinating care plans, authorizations, and ensuring that services align with covered benefits.

Qualifications

• Bachelor’s degree or equivalent experience.
• 3 years’ experience in healthcare/customer service or relevant experience.

Responsibilities

• Plays a lead role in navigation of care and explains program benefits to patients.
• Sets goals with patient and care team to coordinate care and tracks follow ups.
• Ensures provider orders are authorized and within covered services.
• Acts as primary contact to educate patients on pharmacy benefits and prior authorizations.
• Organizes correspondence and creates next steps plans in EMR and benefits systems.
• Provides extensive patient navigation between program visits and encounters.
• Educates patients on program benefits and contributes to process improvements.
• Other duties as assigned.
