Responsibilities

• Plays a lead role in navigation of care and communicates program benefits to patients.
• Sets goals with patient and care team and tracks follow-ups and appointments.
• Ensures orders placed by providers are authorized and fall under covered services.
• Educates patients about pharmacy benefits and assists with prior authorizations when applicable.
• Organizes patient program plans and next steps in the EHR and benefits systems.
• Facilitates navigation between program visits and other encounters, addressing concerns about coverage.
• Educates patients to understand program benefits and services offered.
• Provides recommendations for process improvements to enhance patient experience.
• Performs additional duties as assigned.
