Job summary

Humana is seeking a Patient Services Coordinator to manage visit schedules and coordinate with field staff, patients, and physicians to ensure continuity of care. The role also supports day-to-day office and staff activities, emphasizing clear communication and efficient scheduling.

You will handle agent schedules, backfill for absences, and assist with case conferences, logs, and transfers, with potential on-call duties depending on clinical needs.

Qualifications

• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsibilities

• Manage schedules for all patients and reassign as needed.
• Initiate infection control forms and file with HRD.
• Serve as backup during lunch hours and assist with weekly case conferences.
• Maintain the hospitalization log and share updates with staff.
• Verify visit notes in scheduling console.
• Assist with internal transfer of patients between branch offices.
• Process reassigned and rescheduled visits.
