Job summary

UW Medicine Primary Care at South Lake Union is seeking a Between Visit Care Coordinator (PSS) to support patient access and care coordination. This full-time, 40-hour position handles multi-line calls, triage messages in MyChart, and routes inquiries to the appropriate care team, ensuring timely responses in line with clinic protocols.

Qualifications

• High school graduation or equivalent AND One year of healthcare, administrative, or customer service experience OR An equivalent combination of education/experience.

Responsibilities

• Responds to patient messages in MyChart and manages telephone encounters per protocol.
• Routes calls and MyChart messages to the appropriate care team.
• Acts as liaison between providers and other UW Medicine entities and departments.
• Performs audits of open telephone and MyChart encounter reports and communicates findings.
• Supports referral process with faxing referrals and scanning related paperwork.
• Assists with forms for providers, patients, insurance, and employers.
