Job summary

Reliant Care Management Company seeks a Resident Care Coordinator to provide daily care, assist with dining, respond to call lights, and document activities. You will engage residents to foster a positive environment and coordinate care with a rehabilitation focus while guiding staff for high-acuity residents.

Ideal candidates hold an unencumbered LPN or RN license with long-term care experience, strong assessment skills, and a commitment to Total Quality Management.

Qualifications

• Perform resident assessments and maintain accurate documentation
• Collaborate with team members to deliver excellent care for elderly residents

Requirements

• Unencumbered LPN or RN license
• Previous long-term care experience
• Strong leadership skills and commitment to Total Quality Management
• Excellent assessment and documentation skills
• Passion for working with the elderly
• Experience with behavior-challenged residents strongly preferred
