American Medical Administrators MGMT Company in Missouri is seeking a Care Plan Coordinator to assess patient needs and collaborate with interdisciplinary teams to develop individualized care plans. You will conduct evaluations and ensure compliance with Medicare guidelines while supporting nursing staff with care practices and documentation.

Ideal candidates hold an RN or LPN license with 2–5 years in long-term care, and demonstrate knowledge of Medicare regulations and quality management

Qualifications

• Unencumbered RN or LPN license.
• 2–5 years in long-term care.
• Knowledge of Medicare regulations.
• Commitment to Total Quality Management.
• Working knowledge of Case Mix Index, Quality Measures tracking, QRP, and Value Based Purchasing.

Responsibilities

• Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Perform utilization reviews to ensure appropriate resource allocation and adherence to Medicare regulations.
• Maintain accurate medical documentation and records in EHR systems, ensuring HIPAA compliance.
• Provide guidance and support to nursing staff regarding patient care and documentation practices.
