Job summary

American Medical Administrators LTC seeks a Care Plan Coordinator to join our care team. The role involves conducting comprehensive assessments, developing individualized care plans with interdisciplinary teams, and ensuring HIPAA-compliant documentation.

The ideal candidate holds an unencumbered RN or LPN license with 2–5 years in long-term care, deep knowledge of Medicare regulations, and experience with QA programs including Case Mix Index and Value Based Purchasing.

Qualifications

• Unencumbered RN or LPN license.
• 2–5 years of experience in long-term care.
• Knowledge of Medicare regulations.
• Commitment to Total Quality Management.
• Working knowledge of Case Mix Index, Quality Measures tracking, QRP, and Value Based Purchasing.

Responsibilities

• Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Perform utilization reviews to ensure appropriate resource allocation and adherence to Medicare regulations.
• Maintain accurate medical documentation and records in EHR systems, ensuring HIPAA compliance.
• Provide guidance and support to nursing staff regarding patient care and documentation practices.
