Job summary

Woodsfield Health Care Center seeks a Resident Care Coordinator to provide daily resident care and coordinate a rehabilitation-focused team. You will respond to call lights, assist with dining, and document care activities while promoting a positive environment.

Ideal candidates hold an LPN or RN license with long-term care experience and demonstrated leadership. Senior staff support and high-acuity resident care are integral parts of this role at our Ohio facility.

• Provide leadership for staff while supporting high-acuity residents
• Ensure quality management practices are followed
• Perform resident assessments and maintain accurate documentation
• Collaborate with team members to deliver excellent care for elderly residents

Requirements

• Unencumbered LPN or RN license
• Previous long-term care experience
• Strong leadership skills and commitment to Total Quality Management
• Excellent assessment and documentation skills
• Passion for working with the elderly
• Experience with behavior-challenged residents strongly preferred
