Reliant Care Management Company seeks a Resident Care Coordinator in Sedalia, MO. This role focuses on guiding daily resident care with a rehabilitation emphasis, leading staff, and ensuring high standards of quality and documentation.

The ideal candidate holds an unencumbered LPN or RN license and has long-term care experience, with strong leadership and dedication to elderly well-being. Prior work with behavior-challenged residents is preferred.

Qualifications

• Unencumbered LPN or RN license.
• Long-term care experience is preferred.
• Strong leadership and commitment to Total Quality Management.
• Excellent assessment and documentation skills.
• Passion for working with the elderly; behavior-challenged residents experience preferred.

Responsabilités

Job Responsibilities and Duties:

• Provide daily care to residents

• Assist with dining needs

• Respond promptly to call lights

• Chart and document resident care activities

• Engage, converse, and interact with residents to promote a positive environment

• Lead and coordinate resident care with a rehabilitation focus

• Provide leadership for staff while supporting high-acuity residents

• Ensure quality management practices are followed

• Perform resident assessments and maintain accurate documentation

• Collaborate with team members to deliver excellent care for elderly residents

Requirements:

• Unencumbered LPN or RN license

• Previous long-term care experience

• Strong leadership skills and commitment to Total Quality Management

• Excellent assessment and documentation skills

• Passion for working with the elderly

• Experience with behavior-challenged residents strongly preferred
