Job summary

Oakview Health & Rehab is seeking a full-time Admissions Coordinator to manage pre-admission and admission paperwork, coordinate with families and referral sources, and ensure a smooth patient transition into our center.

You’ll support the interdisciplinary team, maintain timely communication, and help develop outreach plans to connect with partner organizations. Strong organizational skills and knowledge of healthcare navigation are essential.

Qualifications

• Bachelor’s degree in Business Administration, Social Work, Marketing or related field.
• Associate’s degree in related field with three years experience.

Responsibilities

• Coordinate pre-admission and admission processes with patients/families.
• Verify room readiness prior to admission.
• Maintain contact with patient families for smooth transitions.
• Provide frequent visits to new admissions for a positive patient experience.
• Orient the patient to the center environment.
• Participate in the center’s IDT to address navigation needs.
• Coordinate back-up plans for outreach, tours and admissions.
• Develop partnerships with healthcare partners for appropriate care.
